rewari boy (C 5,6 ) injury


He is a 16 year old boy from rewari who father was working in indian railways. He sustained brachial plexus injury in a motorcycle crash. It was upper trunk injury .with a preserved hand functions. After 7 months of injury patient was planned for surgery. After exploration supra scapular nerve was found to be damaged along with upper trunk .Three nerve transfer were planned and done .
Supra scapular to spinal accessory, oberlin transfer and somsack transfer were done. Reason I posted this video is through movement is less,but patient was happy. Continuous physiotherapy will improve movements. These nerv transfer are both extra plexal and intra plexal. But all are close target neurotization . Oberlin transfer is transfer of one fascicle of ulnar nerve to biceps branch, or  fascicle of median nerve to brachialis branch . Somsack transfer is transfer of branch of long head of triceps to axillary nerve.
 Pronator contracture in these patients as result of extensor weakness is pretty common. It is managed with botox injections or surgical work.
