On exploration of brachial plexus axillary nerve was found to be injured ,rest was normal. So decision was taken to to do Somsack transfer was taken . A fascicle of radial nerve supplying long head of triceps was taken and coapted to axillary nerve . After six months patient developed this movement which will improve with physiotherapy.
This is known as close target neurotization where near by good nerve are taken to neurotize damaged nerves. Often a single or two fascicle is taken , being close by it recover faster and reliably. As the transfer is closer to neuromuscular junction recovery is faster. It is considered as distal nerve transfer
