FFMT

A two year old result of free functioning muscle transfer done in 2016. Today 2/3/2018 patients sends me his video of movement achieved. He has reasonable elbow flexion thats is barely good enough for normal person , but these people are remarkably adept with these movements. They make best use of it. Free functioning muscle transfer FFMT is a complex procedure devloped for situations where no other simpler alternative is available. Aim of the surgery is to restore function which has been lost due to trauma or disease.

Free functioning muscle transfer is done in a palliative reconstructive setting in order to achieve specific aims in brachial plexus injury.Doi in japan popularised two transfer which were known as doi 1 and doi 2. Doi 1 is for elbow flexion and finger extension. The gracillis muscle is fixed to clavicle ,pedicle is joined to thoracoacromial artery and cephalic vein. Nerve is joined to Spinal accessory nerve. It takes almost one year to achieve results. Doi 2 transfer done for finger flexion , gracillis muscle is fixed to 3rd rib. Vascular pedicle is anastomosed to thoracodorsal vessels and nerve is coapted to 3,4 intercostal nerve. Wrist arthrodesis and thumb arthrodesis are supplementary procedure required.
This exercise restore basic prehensile function of hand.
